Original Contribution Effect of Trajectories of Glycemic Control on Mortality in Type 2 Diabetes: A Semiparametric Joint Modeling Approach

چکیده
Data on the effect of trajectories in long-term glycemia and all-cause mortality are lacking. The authors studied the effect of trajectories in long-term glycemic control on all-cause mortality in patients with type 2 diabetes. A cohort of 8,812 veterans with type 2 diabetes was assembled retrospectively using Veterans Affairs registry data. For each veteran in the cohort, a 3-month person-period data set was created from April 1997 to May 2006. The average duration of follow-up was 4.5 years. The overall mortality rate was 15.3%. Using a novel approach for joint modeling of time to death and longitudinal measurements of hemoglobin A1c (HbA1c) level, after adjustment for all significant baseline covariates, baseline HbA1c was found to be significantly associated with mortality (hazard ratio 1⁄4 2.1, 95% confidence interval: 1.3, 3.6) (i.e., a 1% increase in baseline HbA1c level was associated with an average 2-fold increase in mortality risk). Similarly, the slope of the HbA1c trajectory was marginally significantly associated with mortality (hazard ratio 1⁄4 7.3, 95% confidence interval: 0.9, 57.1) after adjustment for baseline covariates (i.e., a 1% increase in HbA1c level over 3 months was associated with a 22% increase in mortality risk). The authors conclude that a positive trajectory of long-term hyperglycemia is associated with increased mortality.

متن کاملEffect of Different HIIT Protocols on the Glycemic Control and Lipids Profile in Men with type 2 diabetes: A Randomize Control Trial
Objective: : High intensity interval training (HIITs) can induce weight control, lowering blood pressure and beneficial effects on cardiovascular health in type 2 diabetic patients. The effect of different volumes of these exercises is unclear in type 2 diabetic patients. The aim of this study was to compare the effect of low volume and high volume of short-term intensive training on glycemic i...
متن کاملLipid Profiles and Lipid Oxidation in Type 2 Diabetic Patients with Good Glycemic Control
OBJECTIVE: The aim of this study was to evaluate the effect of good glycemic control on serum lipids levels and lipid peroxidation, and to find out the relationship between the level of malondialdehyde and HbA1c in type 2 diabetes. MATERIALS AND METHODS: Fifty type 2 diabetic patients aged 40-60 years with the history of diabetes for more than 10 years were studied. Glycemic control was stable...

Background & Aim: Type 2 diabetes is one of the most common endocrine disorders, which can include Lack of blood sugar control, multiple cardiovascular problems, and decreased cognitive function . The aim of this study was to investigate the effect of a combined exercise course physical fitness, cognitive function and glycemic control in women with type 2 diabetes. متن کاملEffect of Poor Glycemic Control in Newly Diagnosed Patients with Smear-Positive Pulmonary Tuberculosis and Type-2 Diabetes Mellitus
Background: There is growing evidence that diabetes mellitus (DM) is an important risk factor for tuberculosis (TB). A significant number of DM patients have poor glycemic control. This study was carried out to find the impact of poor glycemic control on newly diagnosed smear-positive pulmonary tuberculosis patients with type-2 diabetes mellitus in a tertiary care hospital. Methods: In a hospita...
